By engaging our services, you acknowledge that you have read, understood, and agreed to the terms outlined in the following policies.

On booking your first consultation, you will be emailed a New Client Intake Form (as well as a Telehealth Consent for online bookings) that must be completed, signed and returned to us no later than 48 hours before your scheduled appointment. For clients under 18 years of age, the New Client Intake Form and Telehealth Consent must be completed by a parent/guardian, and a parent/guardian must be present at each consultation.
All online/telehealth appointments must be paid in full at time of booking. You are responsible for paying either before or at the time of your appointment for in-person consultations.
You must provide at least 24 hours notice to cancel/reschedule an appointment. Appointments missed (no-shows) or rescheduled without 24 hours notice will incur the full fee and must be paid before the subsequent consultation. If you are late for an appointment your session may be less than 75 minutes in duration (or 45 minutes for a follow-up). Please note, you will need to pay the full amount in these circumstances.
Cancellation of an online/telehealth appointment, without rescheduling another appointment, will incur a $10 cancellation fee to cover our online payment system costs.

For the health and safety of clients and the practitioner, all surfaces are sanitised between appointments. The practitioner’s hands are washed before and after each consultation.
If you are unwell with a communicable illness, we request that you reschedule your appointment to a time when you are well again, or request your appointment be altered to an online (telehealth) consultation.
The New Client Intake Form requests a brief medical history and asks you to disclose any medications (pharmaceutical, nutraceutical or supplements) you are taking. Please provide accurate information so we can reduce the risks of a prescribed treatment causing any adverse drug-nutrient reactions. It is your responsibility to inform me of any changes in your health status or medications you are taking while being treated by Happy and Well Nutrition.
I abide by the Scope and Standards of Practice of ANTA, am first aid qualified, police cleared and have a current Working with Children Certificate.

All online/telehealth appointments must be paid in full at time of booking. Online appointments missed or rescheduled without 24 hours notice will NOT be refunded.
You are responsible for paying consultation fees for in-person appointments either before (optional) or at the time of your consultation. Payment can be made online via our Client Portal or using direct bank transfer, cash, debit card or Mastercard/Visa (manual input of debit/credit card details will be required for payments - EFTPOS facilities NOT available).
We offer a 20% discount on consultations with Pensioner Concession, Health Care Card or for full-time students (discount does not apply to Beginner Bundle). Evidence of such concession must be presented to the practitioner at time of payment.
Clinical nutrition consultations are not covered by Medicare, however rebates are offered by many private health insurance companies (subject to your individual cover/policy). Happy and Well Nutrition is a registered provider with: APIA, AHM, CUA Health, GU Health, HBF, Latrobe Health Services, Medibank, Mildura Health Fund, NIB, Phoenix Health Fund, Qantas Health Insurance, St Luke’s Health, Suncorp and Westfund Health Insurance.
In addition to scheduled appointments, my practice is to charge on a prorated basis for other professional services that you require such as in-depth reviews of laboratory/functional testing, bioelectrical impedance analyses, telephone conversations that last longer than 15 minutes, or the time required to perform any other service which you may request of me. I will inform you of any additional costs prior to the service being carried out.

At Happy and Well Nutrition, we value and respect your privacy. This Privacy and Confidentiality Policy outlines how we collect, use, disclose, and protect your personal and health information. This policy applies to all clients, visitors, and individuals interacting with our services.
1. Collection of Personal and Health Information: We collect personal and health information necessary for providing effective clinical nutrition services and for receiving payment for such services. Each new client will be required to complete a New Client Intake Form before their first consultation (this must be completed by a parent/guardian for clients below 18 years of age). This information will include, but is not limited to, your name, contact details, medical history, family medical history, dietary preferences, and health goals. We only collect information before and during each consult, that is relevant and necessary for your care.
2. Use of Information: We use the collected information for the purpose of assessing your nutritional needs, designing personalised treatment plans, and monitoring your progress. This information also helps us provide you with relevant advice, recommendations, and support tailored to your unique situation.
3. Disclosure of Information: We will not disclose your personal or health information to third parties without your explicit consent, except as required by law or as necessary for your treatment (e.g. sharing information with your healthcare provider with your consent). It is important to note that there are limits to confidentiality in exceptional circumstances including when there is a risk to you or others, or in response to a subpoena or warrant. We do not sell, trade, or otherwise transfer your information to outside parties for marketing purposes.
4. Data Security: We implement industry-standard security measures to protect your information from unauthorised access, alteration, disclosure, or destruction in accordance with the Australian Privacy Principles. Access to your information is restricted to authorised personnel who are trained to handle personal and health data responsibly.
5. Consent: By engaging in our clinical nutrition services, you provide consent for the collection, use, and disclosure of your personal and health information as outlined in this policy.
6. Access and Correction: You have the right to access your personal and health information held by us. If you believe any information is inaccurate or incomplete, you have the right to request corrections.
7. Retention Period: We will retain your personal and health information for as long as necessary to fulfil the purposes outlined in this policy or as required by law.
8. Changes to the Policy: This Privacy and Confidentiality Policy may be updated from time to time. Any changes will be posted on our website, and the revised policy will apply to all information collected after the effective date.

I am a qualified (BHSc Nutritional and Dietetic Medicine) and registered (Australian Natural Therapists Association) clinical nutritionist. I use evidence-based practice to guide my treatment including dietary and lifestyle recommendations and the use of dietary supplementation and/or nutraceuticals when required.
Prior to commencing treatment, I will discuss my proposed treatment approach with you and ensure you understand the rationale for the approach, including any identified risks. I will only provide services that are within my professional capabilities, competence and role as a clinical nutritionist. If treatment needs are identified that are outside of this scope, I will endeavour to assist you to identify alternative suitable providers.
